If you would like to join my website using standard mail, please print, fill out and sign the form linked below; attach a copy of your driver's license or other ID (for age verification purposes); and enclose $20.00 (US funds only) for each month you wish to have access. Allow 7-10 business days for processing. Your account will be created and will expire when your subscription runs out. You will receive one notification of activation/expiration etc, you will not receive any solicitation from myself or any third party. Your identity and personal information will be protected. All information gathered about you will be destroyed 6 months after your subscription expires.

Again the steps are:

1. Print the form. Fill out the information required. Sign the form. PLEASE INCLUDE A VALID EMAIL ADDRESS: We will NEVER contact anyone via standard mail. All correspondence TO you will be through email. We will not disclose ANY information about you to anyone for any reason. Your privacy is our utmost concern.
2. Attach a photocopy of your picture ID showing age, name and address.
3. Enclose $20.00 (US Funds ONLY) for each month you wish to have access. (Check, money order, etc. Make checks and money orders payable to DDAF Enterprises)
